Client Medical Consent, Disclaimer & Studio Terms

By booking an appointment and/or signing a consent form at reception, you agree to the following terms.

ICO & Data Protection

Solid Ink Ltd complies with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.

Personal data collected on consent forms or through bookings is used solely for:

  • Appointment management
  • Health and safety compliance
  • Legal record keeping

Records are stored securely and access is restricted to authorised persons only.

Personal data is retained for a maximum of three (3) years, unless a longer period is required by law.

Solid Ink Ltd is registered with the Information Commissioner’s Office (ICO) in accordance with UK data protection law.

Medical Consent

By signing the studio consent form, you confirm that:

  • You are 18 years of age or over
  • You have truthfully disclosed all relevant medical information
  • You understand that tattooing and cosmetic tattooing carry inherent risks
  • You have been given the opportunity to ask questions prior to the procedure

Medical conditions that must be disclosed include, but are not limited to:

  • Skin conditions
  • Blood disorders
  • Heart conditions
  • Diabetes
  • Epilepsy or seizures
  • Immune system disorders
  • Pregnancy or breastfeeding
  • Allergies
  • Medications, including blood thinners

Failure to disclose relevant medical information may increase the risk of complications and may invalidate aftercare responsibility.

Semi-Permanent Makeup (SPMU)

You understand that results from semi-permanent or cosmetic tattooing may vary depending on skin type, age, lifestyle, medical history, and healing response.

Fading, colour change, patchy retention, or the need for future touch-ups is normal and is not considered a fault.

Risks & Liability

You acknowledge that tattooing and cosmetic tattooing involve the use of needles, pigments, and skin penetration and therefore carry inherent risks.

These risks include, but are not limited to:

  • Infection
  • Allergic reactions
  • Scarring
  • Swelling or redness
  • Poor healing due to aftercare, lifestyle, or undisclosed medical conditions

Solid Ink Ltd, its artists, and staff shall not be held liable for complications arising from:

  • Undisclosed or inaccurate medical information
  • Allergic reactions to pigments, metals, or antiseptics
  • Failure to follow aftercare instructions
  • Individual healing responses
  • Factors beyond the reasonable control of the artist
Aftercare Responsibility

Aftercare instructions are provided verbally and/or in writing.

You accept full responsibility for the care of your tattoo or cosmetic tattoo once you leave the studio.

Solid Ink Ltd cannot be held responsible for outcomes resulting from improper aftercare or external factors.

Photography Consent

You consent to photographs of completed work being taken for portfolio, website, or marketing use unless consent is declined in writing prior to photography.

Deposits & Cancellations

All bookings require a non-refundable deposit.

Deposits secure appointment time and cover artist preparation and lost opportunity costs.

Appointments rescheduled with less than 48 hours’ notice will result in the deposit being forfeited.

No-shows will forfeit the deposit in full.

Deposits may be transferred once only, subject to availability, with sufficient notice.

Insurance & Professional Standards

Solid Ink Ltd operates using industry-standard equipment, sterile procedures, and professional-grade pigments in accordance with health and safety best practices.

All procedures are carried out with reasonable skill and care in line with industry standards.

Clients acknowledge that no guarantee is given regarding healing, colour retention, or final appearance due to biological and environmental factors.

Local Authority Compliance

Solid Ink Ltd operates under the licensing, inspection, and hygiene requirements set by Waverley Borough Council, Surrey.

Jurisdiction

This agreement is governed by the laws of England and Wales.
